ESA accommodation request form for housing
There is a $25.00 fee for each accommodation request form completed and signed by our LMHP. Please call us if you have additional questions regarding the accommodation request.
Please note that California has a 30-day waiting period for ESA letters. California law AB 468, adopted in 2022, requires state residents to have a 30-day relationship with a licensed mental health professional (LMHP) before they can be provided with an ESA letter.
